Psychiatry as a Cultural Product

The development of a pan-human nosological framework for interpreting psychiatric disorders is fraught with complications, particularly when the sanctioned medical illnesses of a dominant culture fails to describe the maladies of less dominant or marginalized cultures. This effect is nowhere more obvious than in western psychiatry’s inability to properly deal with culturally-bound syndromes such as Amok, Koro, Latah and Taijin Kyofusho. The classification of clinical conditions within western psychiatry is largely dependent on symptom analysis. Discreet mental illnesses such as schizophrenia, bipolar and depression are interpreted via symptom clusters that constitute a given mental disorder which leads to diagnosis and treatment.

This smooth, linear progression from symptom analysis to treatment falls to pieces in the face of highly variable cultural understandings of illness which may not fit with pre-existing western ideas of sickness and health. Psychiatric disorders are not static entities devoid of social influence. They are dependent on the time and space within which the disease arises. What is constituted as a psychiatric disorder is negotiated through social and cultural networks of meaning. The reduction of psychiatric disorders to purely biological mechanisms that give rise to discreet psychological maladaptations is itself a product of cultural influence and networks of meaning spanning multiple social spheres.

Thomas Szasz has a monumental book, titled "The Myth of Mental Illness" that deals with many of the points you have made.

i'm reading his wiki page now. it's very interesting.

I could reduce this to a nature/nurture argument and then say its a mix of both and some people's maligned mental state might have more to do with genetics and some people's state might have more to do with experience.

imo cultural factors allow for the expression of different genes but that's not really what the op is about.

I think that cultural influences make psychiatric disorders manifest in different ways. Disorders such as phobias, ocd, depression, etc. are all common to human beings but manifest differently depending on stuff.

I personally believe schizophrenia is a process of opening doors in the brain/mind. The psychotic disturbances result when either the psyche is too undeveloped for the new inner horizons or when the door gets stuck, comes off the hinges or, forgive my metaphor, needs oil.

Schizophrenia has revolutionised me. My imagination spills into my perceived world now, but in a safe, harmonious and functional way. Instead of full-blown hallucinations, tin-foil hats and the like, I get this change in the brightness of colours, a slightly different palette altogether, I see how people are each in their own worlds, I don't see the vague rat-race depressed blur I used to see.

I am constantly evolving and schizophrenia feels like it has been an evolutionary step.

i'm actaully writting an essay on the subject at the moment. here's a peice of it you might find interesting.

If we are willing to doubt the objectivity of culture-bound syndromes then we should also be willing to doubt the objectivity of western psychiatry’s culturally validated mental disorders. Tseng contends that culture related specific syndromes are to some degree dependent on larger socio-economic circumstances in relation to the frequency of distribution among a specific population and points out the interesting social phenomena of women fainting at the realization of some traumatic event. This socio-medical phenomenon has almost disappeared with the rise of feminism, women’s rights and the cultural conceptualization of women as strong, independent persons. If a woman’s public fainting can be considered a culture-bound syndrome then where should we choose to draw the line? Any peculiar behaviour could be categorized as a mental disorder. What western psychiatry deems a scientific analysis of human behaviour is in-fact a social judgement call.

Schizophrenia is simply a collection of symptoms that are judged to be maladaptive. If an individual fails to drive their automobile in a safe manner should psychiatrists address their mental health? It is clear that the mental disorders designated culturally-bound are equally ambiguous and socially constituted as those validated by western medical practices. Essentially there are no discreet mental disorders culturally-bound or otherwise. Only symptoms, the expression and diagnosis of which are culturally mandated. This is not a denial of the suffering of those people who have been diagnosed with psychiatric conditions, nor is it a denial of the existence of mental disorders. It is simply an observation that the degree with which we separate mental disorders into discreet categories, schizophrenia vis-a-vis bipolar disorder, is a product of widely held beliefs about the nature of illness. The words of Thomas Szasz (1974) illustrate the arbitrary nature of schizophrenia as a clinical condition poignantly. "If you talk to God, you are praying; If God talks to you, you have schizophrenia. If the dead talk to you, you are a spiritualist; If you talk to the dead, you are a schizophrenic" (Szasz, 1974). Szasz even goes as far as saying that western psychiatry is the modern secular religion! Complete with its own rituals and mythology that he calls “pharmacomythology.” Szasz goes on to claim that “since theocracy is the rule of God or its priests, and democracy the rule of the people or of the majority, pharmacracy is therefore the rule of medicine or of doctors” (Szasz, 1974).

Most human beings self-medicate and what form this takes is likely culturally bound too. But I suspect there is something deeper than culture in both self-medicating and psychiatric medicating. And it's been around for as long as we know.

are you talking about our relationship with chemicals found in our environment and how humans have harnessed them? there is a theory that using drugs is actually a search for more neuro-transmitters to allow for more adaptable cognitive functions.

I was talking about our relationship with suffering and how everyone finds different ways to cope. There is no correct or final answer on how to deal with the woes and trials of life as far as I can tell, or we'd all be using that way I suspect. But there does seem to be more effective or less effective means for each individual. And that is up to the individual to determine this.

Agreed that psychology and psychiatry are Culturally constrained.
Language, tradition, and belief, are keeping us in the dark ages.

I don't think that any cultures on the whole have ever been better at this, allowing for population(, which is a force in itself towards disease).

I.E. smaller societies may have seemed more efficient with their mumbo jumbo than our current one, simply because of the tightness of the groups, and the effectiveness in eliminating dissenters and disbelievers.

I hope our mumbo jumbo evolves so that as we go crazy we fall into capable hands.
