Corpse Finder
Hebeloma syrienes

"This species once brought a crime to light when human remains were found beneath it; In an instance, the mushroom was discovered growing out of a buried box containing a baby's bones. A related Japanese species is known to grow from the remains of animal carcasses"

National Audubon Feild guide page 626.

I have heard that Paxillus involutus also likes to grow near corpses, but was unable to confirm this with a google search.

Funny you mention Paxillus involutus...  Yesterday I found one at a cemetery, and today while i was hunting for birch boletes at school, i kept finding them.

I picked about 5, and then threw them at my friend, i'd pull some apart into little pieces and throw them at him.  He thinks mushrooms are gross.

There is also an X-Files episode that the serial killer grows mushrooms from the bodies of his victims. He puts the bodies in big shelfing trays similar to how Button Mushrooms are cutivated in bulk. And also cased with soil. The mushrooms in the show looked strange tho, i think they made all the shrooms out of grey clay..
