I don't have a reference off the top of my head, but...

During the Cold War, we had a plan of targets to hit with nuclear weapons in the case of all-out war with the Soviet Union. It was our policy to not target civilians, only targets with military importance.
These included 5 within Moscow, bridges in the middle of cities, etc...
