But yeah, most cancers increase in incidence with age.  People are living longer than before and hence, more people are dying of cancer than before.



I should also mention that cancers in general are only recent discoveries in diagnosis.  Prior to the early 20th century, most cancers weren't even known or discovered.

Probability increases with age and for that reason longer lifespans may account for some increase in incidence, but you can not ignore the fact that lifestyle changes are likely responsible for a big chunk as well. Obesity and cardiovascular disease seems to be occurring in younger and younger people and shares fundamental origins with cancer so we can likely expect to see an increase in the incidence of early cancers over the next few decades as well

Yes I agree with that.  There's been several studies linking accelerated tumor growth related to fat tissues in the body.

Common origins are probably a shitty lifestyle. But I was emphasizing that one doesn't cause the other.

Also, people live longer in general and this will lead to higher rates of cancer (but not higher rates in younger people).

Certain types of cells do not cause other cells to get cancer; it is caused by a mutation in a certain part of the DNA in the cell.

It can metastasize though, and also lead to other problems in the body (but not cancer).

That's a pretty loose correlation.  Most major fat metabolism and storage disorders are congenital in nature.  You can argue that any kind of chronic inflammation itself increases the risk of developing malignancies.  But alot of genetic derrangements are usually kept in check by mechanisms such as p53 or the upstream KEY cellular mechanisms.

It's when these collapse that you get in trouble.

I'm very aware of the implications of p53 on cancer progression but the genetic approach does not fully explain the incidence of cancer. ROS generation by leukocytes can lead to genetic aberrations. From there growth factors stimulate tissue growth and angiogenesis (again immune functions). Then matrix metalloproteinases and chemokines remodel the extra cellular matrix and direct metastasis respectively. Then you have the loss of immunosurveillance in chronically inflamed states. Cancer is much better though of as an inflammatory disorder. Genetic alterations play a key role, but without inflammation your likelihood of cancer plummets if not disappears. This is why obesity, cancer, autoimmune diseases, etc are all correlated. They are all inflammatory diseases. Once you have low grade systemic inflammation you are vulnerable to any array of health problems dependent on genetics and environmental specifics.

I don't think I'm disagreeing with what you're saying.  Alot of cancers originate from chronic inflammatory states.  But you can't generalize ALL malignant etiologies as originating from inflammation.  Alot do, but not all.

Some do occur simply from genetic inheritance of certain mutations.  Like Li Fruameni syndrome, or people with inherited BRCA mutations.


My original point is the incidence of many cancers increases with time and age.  Not just from collapses in genetic regulators, but also, as you pointed out, numerous repeated episodes of things such as inflammation.


People were originally questioning WHY there's been an increase in cancer incidence.  I was simply saying because it's a disease of age, and that collapses in genetic mutation regulators was an example.  Chronic repeated inflammation is another example, for sure.

I'm not familiar with the Li Fruameni reference but regarding BRCA... This is a predisposition not genetic determinism. Anyways, I can't "prove" that all malignancies rise from chronic inflammation but when you understand its role in cancer development it is almost implied as a necessary aspect. Almost all the actions of cancer are not independent strategies but responses of a more "stem-like" cell to inflammatory stimuli. we have been acquiring chronic inflammation since the birth of civilization. When we consider the increase in cancer rate we can not just call it a disease of age, but must recognize the rapid rise in chronic inflammation since the industrial revolution as well and the correlations to increases in metabolic syndrome, allergies, and autoimmune diseases. This has had far more impact than our "longevity" and each succeeding generation is more vulnerable to these diseases than the previous and at earlier ages

Yes I'm not ignoring diagnostic improvements in the past hundred years but we have mechanistic studies to support that increase is an inevitable consequence of the course of social change. Age increases your odds, but calling it the greatest risk factor is deceiving. Cancers along with other inflammatory disease risks drastically fall as you move out of urban scenarios and towards rural societies and more so towards hunter gatherer societies. Partially due to the decrease in environmental carcinogens as well as dietary habits but also largely to sanitation standards that alter proper immune system development from an early age. Blaming age and diagnostics for the perceived increase risk is a cop out when the pathphysiology suggests that the rapidly altered environment is making huge contributions. People want to desperately think all this innovation has help us "progress", but the fact is we have removed many of the environmental factors to which our health evolved, in this case the appropriate microbial environment. This effect is amplified each succeeding generation as microbiome and epigenetic alterations to immune function are passed from mother to child. While cancer is mostly an old age disease now I would not at all be surprised to see it common among children in a matter of generations just as with early onset allergies and autoimmunity. So yes, I'm arguing age is not the greatest risk factor. Control for proper immune development and exposure and I guarantee that the risk of cancer at all age groups would be significantly diminished. Age will only be the greatest risk factor if metabolic and inflammatory disturbances are accounted for.
