The problem with a completely free healthcare system is that you have tons of people waiting months and months to see the doctor over asinine shit. When healthcare is completely free people start going to see the doctor over ridiculous shit, such as a minor headache or a cold, which creates a huge backlog and can take somebody who truly needs medical care months and months before you see a physician. This is one of the big problems in a country that has free medical care.

American medical care on the other hand, while the most technologically advanced, is too damn expensive. Insurance doesn't cover everything, many people have high deductibles, and the #1 biggest cause for bankruptcy in America is out-of-control medical costs. A 2 hour hospital visit can cost you $4,000. 4 weeks in the hospital costs you $150,000. If insurance doesn't pay for most of it, you're fucked.

As far as efficiency goes in terms of a healthcare system, Japan has the best medical care system hands down. It's heavily subsidized, so even without insurance a doctor's visit only costs you $30.

Also in Japan, the system is set up so that the physicians already know what your copay is when you give them your insurance card. There's no billing later to your address or any of that shit. They will swipe your card and tell you, "You owe $5 or $6" for this visit. And yes, an office visit actually costs only FIVE BUCKS with insurance in Japan.

It's also not totally free so there are no backlogs or wait times whatsoever.

Japan.... ah, such a truly efficient society.

This has never been a problem for me. It can be tricky to find a GP who is accepting new patients, but I can go to a walk-in clinic with no appointment and usually be seen in well under an hour. If you go to an emergency room with a non-emergency issue then you can expect to be waiting for hours and hours, but that's as it should be IMO. Otherwise it would not be possible to keep space open for real emergencies.

The multi-month waits you do hear about tend to be for things like hip surgeries and so on, i.e. situations where the surgery is needed but the timing of it is not crucial.

That said, some people undoubtedly to come in with stupid complaints and it probably would make sense to introduce reasonable user fees.

They probably should also tie subsidies to medical school to actually practicing medicine in the country for a certain period. You shouldn't be able to become an MD at a discount and then fuck off to the US to increase your earnings. Anyone who does that should owe the full unsubsidized amount that international students would pay.
