Humidity can be a factor here - not enough and they take forever to form (if ever). Too much can damage them, and they will take forever to form (if ever).

Honestly, I'm not aware of the exact Rh in the terrarium. My cat judges humidity by condensation on the terrarium walls - he sprays them once when he puts his perlite in, and if he sees dry spots on the walls he adds more water to the perlite and mists again. It's not very exact, but it works wonderfully for him. He uses an air pump for air exchange (in addition to regular maintenance, photography, and harvesting), and has his stone run into a quart of 10% H2O2/Water.

Almost forgot - the fruiting temp of his terrarium fluctuates between 75 and 80 degrees.

Condensation depends on the temperature difference between the inside and outside of the terrarium. The walls need to be cooler than the air inside in order to see condensation, so the lack thereof doesn't mean it's not humid. Get a good guage.

Your perlite should be damp, but without any appreciable standing water. If youy stick your finger all the way to the bottom of the perlite, the most you want is about 1/2" of water standing. If it's too wet, water won't evaporate properly. Many people say that you should just thoroughly wet the perlite in the bag before adding, with no standing water in the terrarium at all. Your call, really.
Check your terrarium for air leaks. Small holes won't be a problem, really, but ill-fitting lids or large holes could lower your Rh a bit.
Finally, if all that fails, use a quart jar of 10% H2O2/Water with an airstone run to it. This not only raises humidity a little bit, but also provides for good gas exchange.

I would think that it would be unusual for a terrarium to be cooler inside than out. The condensation on the lid does evaporate from my cat's terrariums after the light has been on a while, but first thing in the morning the thing is all foggy.
I guess you're right, though - condensation isn't a sure way to tell your humidity. Seems to work well for kitty, though! I guess it's just a matter of getting a feel for your personal setup.

Funny thing... I have one tub in the bathroom with wet perlite on the bottom. The wall of the tub that faces the door (where the a/c comes in) gets covered in condensation, the wall facing the bath tub is relatively dry, the end walls are semi-coated in moisture. The guage inside reads 96%.
