I had a FOAF who's DT (Detox) story I heard about... this is all hearsay but allegedly
My FOAF's mom was a hardcore alcoholic. She manged to go cold turkey for a few days and had some vivid hallucinations and experiences.
She started drinking again and got back into her old self so I don't know what is the best way to go.... I'd imagine going to a doctor.. any doctor... a walk in or whatever and then ask about the best way to deal with this.
In fact... I wouldn't think it wouldn't even be that crazy to locate your closest AA meeting and talk to the person who runs the show. I bet he/she has seen a similar situation and can give you better advice.
If you need help looking... I know of a few AA meetings in the past that took place at a church. Odds are they don't take place there until the priest knows about it. So there's one good place to start looking for assistance.
If you're an alcoholic quitting drinking for real and have serious problems... AA is the first place I'd look. It's an organization for alcoholics RUN by alcoholics. Between the members, sponsers, organizers, etc... I'm sure they've seen it all.

Not sure if this is the case here, but alcohol abuse does lead to long-term (permanent psychosis).
This happened to my neighbor, but it took 20 years not 2. He drank so much over the period of 20 years that his brain had actually become deteriorated, looked like swiss cheese from an x-ray.
A gallon of whiskey does seem like quite a fucking bit to be drinking every day, maybe your friend is suffering this psychosis due to brain damage?

I've heard of the 'Hitman' paranoia enough times to think it is a common occurrence that isn't confined to withdrawals.

I've no experience with that sort of psychosis, certainly sounds like his dopaminergenic/GABA/NMDA systems are seriously out of wack. A CAT scan/neurological exam would certainly be in order.
I have done enough research concerning my own alcohol issues to suggest seriously considering a thiamine (B1) supplement and perhaps a good general multivitamin.

Benzos and beta-blockers are used with alcoholism withdrawal treatment but definitely doctors orders only stuff in this case.

Atypical anti-psychotics, Dopamine antagonists, Serotonin regulators, benzos etc... You're not going to get that stuff prescribed to you.
The most commonly referenced (older) anti-psycotics I've come across in the literature referencing alcohol psychosis require monitoring via blood tests.

I've not heard of potassium deficiency directly causing symptoms like this but potassium ions are a major component of cell function. Two other probable deficiencies he would likely have and are potentially serious are B12 and Folic acid.
Again I'm not saying this stuff will relieve his symptoms, but if left unchecked they will cause irreparable harm potentially resulting in a marked degradation of his condition.

Getting your cousin some sort of benzo will help IMMENSELY with his withdrawal and reduce his psychosis. Go see a psyc or doctor asap and let them know the situation. They usually taper alcoholics with benzos because it acts similarly on the brain as alcohol. And because alcohol withdrawal can be life threatening in extreme cases (like your cousins 2 year gallon a day habit) without 'easing' the brain back to sobriety.

Sorry to hear that, sounds rough. Alcohol withdrawal is known to cause delirium and paranoia. I had the same experience from caffeine. DO NOT fuck with caffeine anhydrous!
I knew it was the caffeine but it was still very hard to believe what was happening and what was illogical. Things "linked" together to make sense and this re-enforced my delusions. For example, I might think somebody is going to kill them, and then brush it off and go for a walk, but then I would see somebody running towards me (probably to catch the bus?) and this would register in my head as being somehow linked to the noise I heard at home.
Here's what helped me the most: I remained 24/7 with somebody 100% sober and logical. I basically trusted them with my life, in the sense that, if they don't panic, I have no reason to panic. If they don't freak out run, then I shouldn't either. This was VERY hard but I was able to do it because I knew deep down that I myself was being illogical. I was sitting at home and saw that I was freaking out and running all over the place, but my cat was sound asleep, and he's normally jumpy. So my cat became my baseline until I called a "psychosis sitter" who stayed with me till I was sane. A little less than a gram of caffeine on an empty stomach, lasted around 2 days with the worst physical effects I've ever had, but not worse than alcohol withdrawal.
I'm pretty sure he will get his sanity back. He needs to know that he's believing all these things due to withdrawal. Make sure he eats solid food and sleeps. VERY important!
