Let me preface this statement by saying no one method is the 'right ' method, and that each individual will have to find his/her own right way- That said, I will share the experiences the Temple Priests have had.
They use a .75 " layer of moist, not wet bottom layer of straight verm. 'Moist not wet' is an ambuigious term, so far we have been able to explain it by taking a handful of soaking wet verm, and squeezing it so it drips out. When you open your hand the verm is still wet, but has no drippable water, this is "moist not wet'.
Next they crumble the cakes into even sized chunks. Some ppl cut them, some just rip them apart, ther is no set way. It will all be placed into an EVEN layer across your casing tray/tank.
It is also noted that when the cake layer is put down, it is tamped, or firmly,but not forceably patted down gently into the bottom layer. It can be of a thickness .50 - 1.5 inches deep.
The top layer will be 'sprinkled' enough to cover .50 - .75 of an inch. Never tamp top layer. Some like a thick top layer 1 - 1.5 inches, saying it gives shrooms strengh to bust through,but the .50 - .75 had never failed.
As to how many cakes you can put, it depends on the size of the tank. more is better than less. If you cover you tank from end to end, your good, as long as the cake depth is even. We have a 20 x 10 inch long tank, 9 cakes was just enough.

Well, my casings aren't doing that well yet, but I understand that to mean that 2" substrate = 1/2" casing; 4" substrate = 1" casing. Now that I see myself type it, I realize I haven't done it. Crap. Wonder if that's why I got cobweb?

That is kinda confusing... reworded+example:

The depth of the casing should be close to one quarter the depth of the substrate. For example, a four inch deep substrate would have a one inch deep casing.
