contam probs

I First grew on cakes with a Ultra Sonic humidifier, only did it once. then i switched to perlite and no humidifier, and casings instead of cakes. always plain verm casings. I've been having a problem with them ever sence the switch. After the first flush i usualy wait a week and still dont see any pins of a second flush, but lots of contam. always green mold or cobweb.
I mist the casing whenever they are dry. the perlite never touches the casing or substrate. I use PLENTY of H2O2 in the perlite. My container is a near air tight cooler on top of a heating pad. the temp is about 80 inside the cooler.
Should i dump the perlite and go back to my US humidifier? Should i go back to cakes? both?
Any help would be greatly thanked. I've been growing for over a year geting one flush before contams set in. I sometime am able to battle off the contams for a week or 2. in the end they always win.

It probably isn't getting enough air exchange...
Fan that sucker out at least twice a day...or get yourself a couple fish tank bubblers and run them all day.

With what are you spraying the casings ?
I allways sterilize the water before filling the spray bottle.
