Shrooms growing the wrong way is from light in multiple direction insted of up. This can be helped by painting the inside of your terrarium flat black so that light doesn't reflect around inside. Overall though, there will always be one that will grow the wrong way or twisted or some stupid shit. I'm not sure about the puffy thing your describeing on the caps. Sounds like mycelium on the caps, and if it is, its perfectly normal.

The puffyness is probably the "fat-ass-ness" showing up and is usually due to too high a humidity.
