We all have a seizure threshold. If we move beyond it, we will twitch, move further it gets to seizures, move beyond that and it's extreme seizure leading to injury and to its utter extreme (usually only after ingesting a heavy dose of some kinds of poison or severe head trauma) it becomes like a scene from the exorcist with the body thrashing about, muscles ripping and bones popping that often ends fatally.
Apparently your friend for some reason is closer to his convulsive threshold than he used to be, in times of strong stimulation.
Unless there's a pretty clear reason for it (speed & coke abuse, which then should end) he might want to report this to his doctor as there might be an underlying medical condition that might get worse. If he should be on his way to developing epilepsy for instance you don't want to wait until he gets his first seizure while driving his car.
Many people twitch during very good orgasms. I do But if his sex hasn't suddenly become super, and nonetheless he did develop this recently then a visit to his doctor is in order for good measure.
