i woudl reccomend a lid personally :)
looks ok. From little i knwo about growing mushrooms, only problme i forsee is fan blwoign most of mositure out, which coudl happen if fan on 24/7. A simple timer should solve this, blow for 10 mins every6 hours or something.

I wouldl make the air intake a bit lower, near the perlite, because Co2 (wich is what you want to remove) is a heavier gas than o2. Your idea works.
My terrarium has two holes, i prefer sucking the air out using a vacum cleaner, 1 min. is OK.
