I've been following these since GSK made their 2008 Almorexant announcement.  Both Actelion and GSK have been playing their cards very close to their chest since this drug's development began.  In the industry, that's usually a bad sign.  When clinical trials proceed positively, pharmaceutical companies typically feed their data to medical journals around the world as a kind of pre-marketing strategy.  When they are hush-hush about it, it's either because they have a head start on the development of an entire category of pharmaceuticals, or because they are concerned that worrisome results might sour the drug's reception, dissuading doctors from prescribing it.  Since Mercke has been racing GSK to the finish line with the development of its own orexin antagonist (Surovexant), the latter is the most likely scenario.

Without publishing any specific data (other than a trial summary and press release), GSK abandoned its development of Almorexant a couple years ago.  Pharmaceutical companies do this when their estimated settlement losses exceed their estimated profits.  Last year, Mercke went into damage-control mode following a published review from the FDA about the questionable safety of Surovexant.  Trial patients reported narcoleptic and cataplectic symptoms persisting well into the following days, and several patients reported suicidal thoughts. 

Despite this, Surovexant received FDA approval last August.  What changed in that time?  Who knows.  Perhaps they found a dosing regimen that minimizes these side effects.  Perhaps they added some contraindications.  Perhaps there were some greased palms, or a series of transactional blowjobs.  All of these are possible.  We'll have to wait and see, but until then anyone who takes an orexin antagonist is basically a guinea pig.  As this is a new class of compounds completely without precedent, it remains to be seen how many demons will emerge.

I hope it works out, insomnia is a terrible curse. As far as getting fda approval, just grease the right palms, same as always.

Or complete controlled clinical trials lasting up to a year with thousands of patients to demonstrate safety, and follow the advice of an independent advisory committee.
