Wat? Got a source for that claim? I have an HMO and it was covered completely. Reversal I'm not so sure about.

My great uncle had a vasectomy after they had 3 kids, but they had a 4th kid after somehow. However this happened awhile ago so maybe they've improved vasectomies since then. Just be aware some might get through, maybe talk to your doc about this.

And at #4: women (and/or insurance) have to pay for their daily birth control so it makes sense the tubal would be 100% covered.

My doc said that used to happen. He said the most modern procedure was to cut the tube, burn the two ends, tie off the two ends, and then suture a piece of fat between the two ends so they can't even touch and regrow.

I'm also looking into this, freezing sperm, whether it's reversible etc.

The freezing sperm thing sounds good until you do any research. Didn't look up cost of storing it but if you ever wanted to use it a insemination treatment is $300-700 per cycle and only 8% success rate per cycle.

Sounds like a stupid idea if you even think you might want kids at some point.
