Nutrient deficiency maybe? What conditions are you growing it in?

You say you never changed the soil.......

Did the other cactus die with the same symptoms?

If so maybe some type of poison or chemical was introduced at some time?

How often do you fertilize and at what strength?

the tricheronus species is native to warmer climates, you need to magnify the red and blue spectrums from the light, you can take them out of the light using special paper that is sold in hardwware stores for cheap. Then, you need to use what they tought you in high school about adverse pylons and use a black metal not aluminum as a reflector for your san pedros, maybe you should try some kind of a unichorus cactus instead of trich, as they are mainly for the heavy duty hulticulturists.
