Odd Mutation/Deformation Second Stem out of Caps?

After successfully cultivating my first mushrooms, i decided to try casing a couple of Ecuadorian cakes off of a syringe that I made. I was pleased to see literally hundreds of pins on my substrate but I noticed that over half of them started to mutate/deform. The mutation looks like a second stem is growing out of the cap of the pins. I am using the typical 40/60 Verm/Coca casing, Temps in the 70-75 range, RH of 90-99%, FAE twice a day, and I only used rubbing alcohol to clean the terrarium. I have never seen this until now. Has anyone experienced this or knows how to correct it? Please see pics below.

check it out or search uglies/mutants. happens more than you think. you can fix it by cloning fruits you deem worthy.
