I decided some time ago that the best treatment for my case of frenulum breve was to lengthen the frenulum by inserting a suture, which would kill the the tissue directly underneath the suture and lengthen the frenulum by twice the depth of the suture (hopefully!).
I performed the procedure last night, and thought it might be of interest to people here.

A 2/0 black braided silk suture was inserted. EMLA cream was recommended for topical anaesthesia, but unfortunately in my country it's only available with a prescription, so a simple ice-cube was used to numb the area before inserting the needle. The area was cleaned thoroughly with soap, then alcohol before inserting the needle. There was minimal bleeding. The procedure was moderately painful, mostly due to the disconcerting feeling of inserting a needle through my own tissue. I can certainly imagine that had this procedure been performed by someone else, it would have been at worst mildly painful.
As can be seen from the pictures, the suture was inserted a little lower than the ideal spot. This was due to the awkwardness of suturing myself. I'll see how this turns out, I may well need to repeat the procedure at a later date.
As can be seen, the suture is hidden when the foreskin is allowed to hang naturally forward.
