applying for another job at same place

I am applying for a diffrent job at the same place, a hospital. Now when I worked their last i worked my way up a few possitions and quit. then reaplyed, and went through the hireing process but failed the UA because thc was in my system. now do I put that I previously worked their? or just leave that out?
