Depending on what you put into the smoothie, it can be incredibly healthy. Right now I start off my day with a veggie and fruit smoothie. There are probably about 10 different ingredients in my smoothie, they are either veggie or fruit, there is no proteins and not too many carbs. Then I tie everything together with water. I don't use a base like milk or juice or anything other than water. I want to keep my smoothie as low in calorie and as healthy as possible. I believe that when done right, a smoothie should provide enough minerals and fiber for a day. Another good thing about smoothies is that you will be pairing unpalatable things (such as bitter melon or beets, healthful veggies that nevertheless taste awful) with sweet-tasting things such as strawberry or banana, and this makes it a lot easier on your palate.
