i found this is the faq's.. but i have some questions.
Procedure:
Wet vermiculite and put in sterilizing container, sterilize and let cool.
Sterilize fruiting container and let cool.
Add 1/2" (1cm) or more vermiculite layer to bottom of fruiting container.
--do i sterilize this before i put it in the bottom/wet or dry. ?
Crumble or leave cake whole over bottom layer.
-- 1 cake or 2 cakes ?
Cover cake with damp, sterilized vermiculite (note: some growers suggest leaving crumbled cake without casing until it starts to grow mycelium again (24+ hours))
-- how much should i use to cover it ?
Cover fruiting container with foil or plastic wrap, poke holes in covering.
-- what are the holes for ?
Let stand.

Damp, not soggy.

Your choice. I would recommend 2.

Casing layer should be about 1/4 the thickness of your substrate.
