Do a search for 3in1 or scotts. This subject has been beaten to death lately. You will find an anwser to all your questions and more. I can tell you that it should be pasteurized. Good luck.

i found it to be a bit skimpy on the dung,
you'll want to spawn heavily and/or supplement it with more dung, etc. to get big yields.
colonizes fast, fruits well.
