ID Request Eastern Kentucky Appalachian Mountains

This morning was a nice morning, it had rained two days ago and it's exceptionally humid and warm. Of course, I grab my camera and take a short trek into the mountains. I took quite a few pictures of several different mushrooms in hopes I could get them identified, so I turn to you guys, the experts!
Habitat for all: Growing in the woods of the Appalachian Mountains.
Gills should be very clearly visible.
Stems should be very clearly visible.
Caps should be very clearly visible.
Spore prints are unknown for all.
No visible bruising to my knowledge.
Location for all is the Appalachian Mountains of Eastern Kentucky.

First is likely Pluteus if you found it growing on wood?
Second is Mycena leaiana.
Third is a Lacrymaria species.
Fourth is also Lacrymaria.
Fifth is possibly Entoloma clypeatum if it was growing from soil?Other possibles include Volvariella if they had a volva (these can be on soil or wood) or Pluteus if on wood.
You should include what substrate you find the mushrooms on,that makes the IDing a little easier.

Great picture of those Mycenas. Danny has the right IDs I believe. And I think 3 and 4 are both Lacrymaria velutina. Lacrymaria - tears of mary. Weird genus name.
