OK, so what are the ramifications (upstream and downstream) of everyone in a big city (like New York City) flushing all of the working toilets at the exact same time....?

This is not a toilet related question.
Well, sewers have loads of extra capacity so peoples can get down into them. Even the sewer feed pipes would not be blocked up. If anything, it'll probably purge any heavy bits of flotsam in them.
Now, in terms of filling up, it might take a while for each cistern to refill.

I thought it may cause problems more so with highrise buildings and such, sewers and infeed water problems.... Don't know much about plumbing, that's why I asked....

well, assuming everyone is up to code, there shouldn't be a problem, however, that is not always the case so a few people would probably have some issues but that would have happened anyway.

I saw on TV that right after the tonight show monolog there is a large spike in sewer flow. Can't remember what show it was but it didn't seem to be a problem.

I don't know, but I've heard there is problems w/ some apartment buildings w/ many more toilets than they have sewer line volume for during popular shows' such as the super bowl (no pun intended).
Those on the lower levels get a positive buildup in the sewer lines w/ predictable results.

Yeah, these systems are designed to operate at approx. 150% capacity, so w/o engineering error there shouldn't be a problem.
I'd be curious to know how many toilets in NYC DO flush every ten minutes/ minute/ second. Seems like the kindda useless factoid that should have shown up under a snapple cap by now

if u ever look into a sewer main, it is a very quickly moving river. i bet nyc has rapids in their sewers.
