A few quick questions about casing...

I've looked through several casing teks and from what I learned, I decided to try crumbling three cakes into a foil baking pan and cover it with about 3/4" of miracle grow moisture control. Two days later (today) I checked it out and there is some mycellium breaking through the top in several areas. Now for my questions (I'm new at this mind you, and yes I did search posts for these questions but the teks are quite vague at some points).
Question 1: Should I keep my casing covered until mycellium completely overtakes the casing layer?
Question 2: Should I mist my casing layer and if so, how much/often?
Question 3: After the a flush, should I add another casing layer?
Question 4: When I patch the mycellium that is poking through my casing layer, can I just push uncolonized casing layer over it or should I use fresh casing layer?
[Here's what I'm working with; the humidity is always between 85-95% and the temp ranges from 73-79 degrees. I have 5" of perlite and I mist my fc anywhere between 3-5 times daily]

1) You mustn't have read too much as if you had you would know you don't want your casing to be overtaken by mycelium. It's job is to provide humidity to the substrate, not serve as a growing media. Completely overtaken casing layer will overlay. As soon as you case you should induce fruiting conditions.

3) No, patch at most.
4) Use fresh or you will be thinning out other areas of the casing.
