Contamination... whyy???!!?

This is, so far, the second time I've had to throw out all of my BRF/vermiculite jars. Every time I inoculate them, the white fungus starts to appear, but a green spot appears a few days later in the middle of each inoculation site.
My question is: why? Although this is an extremely general question, I can't help but wonder what are the most common causes of contamination.
I sterilize them, covered in aluminum foil for an hour. Then after they cool down, I simply take the tin foil off, stick the needle in (which has also been sterilized via alcohol flame), inoculate, and then leave the jars in the dark for a week. Once I take them out, their contents goes right into the garbage: contaminated.

Since green molds are killed off at temperatures far below boiling, you can assume they're being introduced at inoculation. If you're using a glovebox or flowhood, and a flame sterilized needle, it sounds as though the problem is in the syringe. RR

RogerRabbit is correct. I'd also like to point out that the white mycelium you are seeing is likely just mold mycelium. The green coloration is from the maturing spores which generally appear first in the oldest areas of the mold's mycelium, which would be the center.

Thanks for the responses. Time to throw this one out, and try syringe #2...

I would suggest that you keep one jar as a "control" jar and don't inoculate it. Do everything the same with that jar but not knock it up. That will definitely exclude the syringe.

That's what I did. Turns out it was the syringe that was contaminated, as I threw it out and used a new one. Now the jars are fully colonized and without contam. Thanks for the advice everyone.
