Mushrooms which have grown from a contaminated cake (either bacterial or moldy), but do not have mold or bacteria growing on them are fine to eat.
Mutant mushrooms are okay to eat.
Moldy mushrooms should be dried and perhaps made into tea before consuming, though, sickness is highly unlikely. I personally just toss moldy mushrooms out... i don't like mold.
Mushrooms with bacterial contamination should not be eaten. Bacteria causes rot, and the mushroom will begin to smell sour, or like trash, or just gross over all, and the tissue will turn black. Eating mushrooms with bacteria on them can cause food poisoning, even if boiled in tea.

For the record. If you have contam on a shroom (of a known to be safe species), you're not going to die of "mushroom poisioning" - (kidney failure). Maybe a little food poisoning, just like we've all had before.
