Yeah, my cooker has an insert that keeps the jars off the bottom of the pot. It elevates the jars about an inch from the bottom. So should I fill it up an inch above the insert?

Err, pressure cookers are not meant to be filled up. And if I did that the jars would be submerged. The function of the water in a pressure cooker is to generate steam, an inch of water on the bottom of the cooker is more than sufficent. I usually use distilled water in my cooker, it helps to keep the mineral deposites off the metal (maybe I've just got hard water, but seem to have trouble with this).Don't leave your jar lids loose in the pressure cooker, there is no reason to do so, and leaving them loose can result in the moisture content of the jars changing (even if they don't fall over). It is not possable to damage the jars with the pressure. Since the water in a sealed jar will also vaporize and generate internal pressure, the pressure differental between the inside and the outside of the jar is nearly zero. If you prefer the lids to be loose during incubation, feel free to loosen them after you have pressure cooked them.

3-4 cups is all I use. I never have any problems, plus it gets to 15 psi faster because the small amount of water boils faster producing steam quicker.
