Are you misting the pins? I think that will cause them to abort like that.

Also, it just may not be hot enough. Mushrooms grow like mad at 80-84 F and fine at 70-80, but I bet they're pretty slow below that. (need to be 65-75 initially to pin?)

Once they're pinning I don't think drying out is an issue for killing the shrooms, though they will get tough-skinned on you.

it sounds like your enviroment just isn't up to par. Spend some time and money on the temp., humidity and then come back. It is hard to say what is wrong b/c you have only prepared a substrate, that would tell me the environment needs to be manipulated...

I agree, we must remember that a shroom is 98% water... No water (I.E. humidity) ='s no growth!
