i shift positions a lot when i read, but i usually lay on my side and hold my head up with my hand...

i some reason like a chair that's real low to the ground, thick cushioned but not plushy... sturdy... and sit indian style with the book resting in my wide open hand which i allow to hang kind of limply at about slightly under face level, but not in my lap, so my head doesn't nod downwards.... back somewhat straight, sometimes in the night i've put a pillow behind my back to keep me more propped up... i dunno, that's just what i do. do you stay on task with other things? i don't know if i recommend it or not, but some people i know claim to have had much success after getting on adderal for ADHD.
