I had the same problem. I know my enviromental conditions were ideal so it was either the casing or substrate depth or just the strains tendancy. I never let it get to a second flush but I have a new casing that is doing the same thing. I also cased with vermiculite only, however, I was using colonized rye as my substrate.

Could be that you purchased your spores from a vendor that produces them from brf/verm cakes. In this case, these strains won't do as well cased as they would on the substrate they are familiar with.
