may actually be too little humidity. fanning 3 times a day in addition to an aquarium bubbler might be excessive, and drying things out. perhaps add some misting to your routine, spray some light mist after you've fanned to replenish the humidity.
have seen it before when RH was too low, the pins come but they just kinda chill near the surface, getting wider around rather than shooting out. increase the RH and voila, they take off (and end up with some funky looking stems as a result of how they started life!) even if they dont go much further they usually end up pretty thick and solid and might surprise you to how much they weigh when dried, so all is not lost even if you cant get 'em going.

Waterlogged substrate. Usually shouldn't happen until after the 2nd flush.
At least that is what PF always said, and it's been my experience also.

PFC's are also known as "Fatasses" it is a strain characteristic along with a veil so thin it disappears shortly after the caps open.
