Failed casings. bad pH? too wet?

Long story short: a friend's casings failed. Perhaps someone has some suggestions on what was done wrong.
Long story:
AFOAF cased about 10 quarts of WBS all told. A few casings with a single quart, a few with 2-3 quarts per casing. 4 different strains, about 5 casings total. Casing material was 50/50 Coco Coir and Peat Moss. He added crushed oyster shells (powder) for pH correction to a pH of 7. pH was checked by adding a few mL of water to a pinch of casing material and dipping a pH indicator strip in said solution. He used quite a bit of oyster shells. To estimate, probably about one cup for a little under 2 gallons of casing material.
Cased about a half inch on top and bottom, then sprayed very liberally with water (not to the point of pooling water, but the casing was quite damp). Cased in a tupperware-type container. Closed shut and left in dark incubator at 85 deg Farenheit. Opened after a few days. Very heavy condensation. Slightly strange smell, faintly sweet and sour, if that makes any sense. Those containers which were totally closed, btw, had puffed up a bit, so one might assume that CO2 was being produced. Containers were reclosed, and opened every few days for inspection. Always the same smell, and it's not a smell he remembers from the last time he cased, though that was over a year ago. Dissected small parts of the casings a few times, and in every single case, the mycelium failed to grow into the casing layer at all.
Anyway, the mycelia never grew. Not even a bit, it seems. It's been about two weeks already, so it seems the casing has failed.
Does anyone have any ideas as to what happened?
My friend figured he'd dig a shallow hole somewhere, fill it with dung mixed with what remains of this project, minus the casing. Comments?

Bacterial contamination would be my guess. Detail the sterilization procedure used for the pans and casing.

You don't case the bottom. 85 is also too hot to incubate cased bulk substrates, so you favored bacteria production over mycelium. Next time, only case the top and incubate at room temperature, or at least not over 80F.
