What are telltale signs of bacterial contamination, and what're some good practices in avoiding it?

Can it be harmful to humans?

the most common bacteria you will encounter is a type of bacillis.. it has a distinct rotten apples/dirty socks stench.

There is more to worry about than just Bacteria. You need to be on the lookout for fungi and parasites as well. There are also a few virus strains, but they are fairly rare. It is easier to know what one thing to look for in healthy growth rather than to know everything to look for in unhealthy growth. If its not white, whispy, with a clean earthy smell, then take no chances and toss it outside in the trash. The two exceptions to this are the blue bruising you will see from time to time, and the dark purple (almost black) spores that are released after the caps open up and mature (You usually pick them before the spores drop to maximize potency). Both of these are typically only seen around birthing and harvest time. Reds, yellows, greens, blues (except as noted above), blacks, "wet spots", etc should all be avoided and tossed. Some contaminates such as Aspergillus, which is very common, can be toxic (A. Flavus, A. Niger, A. Fumigatus) while others are harmless. Don't take chances, if it doesn't look right, don't bother opening it up and smelling. Be safe and get rid of it.
