some say as low as 80% post-pinning. 80-90%, or 85-90% is the general consensus. i do 85-90% myself through the entire fruiting stage and get GREAT results.

yeah, the casing should give off more than enough humidity that way.
you probably have light and temps covered already, thats pretty easy.
now you should have humidity covered.
air exchange = as much as it takes to keep it at 90% or below (it will most likely generate more than enough humidity and you will need to keep it from getting too high) but not so much that it goes under 80%.

Do you think it would be better to fan 3 times a day or use 2-4 holes with polyfill? the temp will be around 70-72 and light will be artificial. so not much heat will be coming from the light source.

i would do polyfil holes, but still watch the humidity. if it starts going over 90, fan it a little. make sure you have a fan in the same room that it is in circulating air. it sounds like everything is great, just watch your humidity and you should be cool.
if you don't have a hygrometer, or one that you can put in the tub, just watch the growth. the mushrooms will tell you whether they need more or less humidity/air exchange. fuzzy stems, et cetera. do a search.
