well, even if this was a serious problem... you don't "fix" a plant, they're not a '67 chevy... they're a living thing . Sorry, I just had to make a quip about that.
As for the spot, it's normal. The growing tip of mine has one, many older healthy san pedro form them. The best I've been able to decide about it is that it may be bruising... but they do not seem to cause any harm to the plant at all. The area tends to be still firm and healthy, and doesnt deform any more. Perhaps we should consider it much akin to a mole on one's skin?
Also, check the search function out, there was a topic on this up here at the top a day or so ago I beleive. No harm in asking, but sometimes you can save yourself the effort.
Keep an eye on your pedro in case worse stuff comes along, but all odds are against this being indicative of a problem.

As far as I know those are a result of an insect parasite. You do need to watch them as they can spread. The insects can burrow through the plant and start new ones elsewhere. Although as konnrade said, they are relatively common and usually not too much of a big deal.
What I do is get some methylated spirits and a large q-tip. Soak the q-tip in meths and rub it on the black spot once a day for a few days. Keep an eye on it. It should stop growing and eventually turn light brown. Don't pick it off, that only damages the cactus more.

hrm... that sounds like a remedy worth trying, I might do it to mine if more show up, just because I like to spoil my baby trichos
but, if I may ask, what are methylated spirits? I've never heard of such a thing.
