bacteria

I keep getting bacteria in my jars it seems. I was just reading the post be mordor and I guess that is why I have lost a few jars. They stall after shaking and get what starts as a light brown goo then it turns the color of the finch seed and stalls the growth. I try and let them strain for a good amount of time and I also pressure cook the jars for an hour but I still have it happen. How can I stop this?

Clean everything with a 10% bleach by volume solution.
If that does not help switch substrates.
