Should it be legal for two consenting adults to fight to the death in a duel? Why or why not?

.... Sure, provided you can provide a format that suggests that both parties are consenting to it under their own free will (contractual)...
Further, provided their are restraints placed (locations for duels).
"Why or why not?" -- Your life to play with, provided you and those your with in said affiliated activities are willingly engaged in them; beyond a doubt. Further, providing no harm to someone else without their will (purposely of course, can be applied to non-humans as well to a degree e.g. other living entities).... Then I truly don't see the issue.
Call it an advanced sport with waivers.... winner takes all.
Although it's archaic (and I don't see the need for it), at least it'd be organized... better then the gang warfare that we currently have, if people could legally settle their disputes in an organized manner, others won't be harmed as a result of it.

Well I don't really have any moral objections to it (two consenting adults can do whatever the fuck they want, just as long as they leave me out of it)...I can see how it COULD turn into something quite horrid - like the days of gladiators in old Rome.
Have you ever played the Unreal Tournament games? I could see something like that growing out of a law which allows consenting adults to duel to the death. It would only be a matter of time before some TV producer realizes the "entertainment" potential of such an act and works to organize tournaments and such.
Shit, have you ever seen a Japanese "deathmatch" fight? Some of them go pretty far (usually ends with one guy in an ambulance, and I've seen one or two where I'm sure one guy died). There's a pretty huge underground market for those kind of things already, and they aren't even legal. Make it legal to kill a man, and it's only a matter of time before we're all cheering on the deaths.

I too would see a huge chance of exploitation.. it would be broadcast just like judge judy. A brief reading of grievances between the parties involved, and a fight to the death...I don't see how it would help anything. America has enough problems as it is, socially speaking...

Well concerning the law, and as it is all just hypothetical...
Laws could be in place to avoid such scenarios by making them private duels, and a form of registration of a contract, and later a designated meeting point in dueling zones, with no forms of communication equipment on site. (just an example)

What if someone dishonors you psychoactive1984? How about your family? It was done in the past, and it proved men to be men. Andrew Jackson was famous for dueling. It separates the men from the boys, you don't have to play.
