I follow the basic PF Tek. (1/4 cup brown rice flour, 1/2 cup vermiculite, 40-45 cc's of water) I guess "pack" wasn't the word I wanted to use. For my first 10 jars I did actually pack the substrate into the jars. I thought that was my problem, so for the second set of 10 jars I just gently spooned the substrate into the jars and didn't pack it or settle it or anything. I sterilized the jars in a big roasting pan with a tight lid on the stove. I haven't had any problems with contaminants, but maybe during the sterilization the amount of water in the jars changed. I don't see how though. I'm pretty stumped by the whole thing. It all works eventually, but it sure takes a long time.

Try to make agar plates from the spores, agar squares speed up colonisation, because spores need to germinate they take longer to colonize. When putting fresh mycelium into jars this will grow on rapidly!!

If you have space in your terrarium put them in there, that should keep things moist (my guess at the problem would be drying. Also don't tape the holes.

What strain are you using? did you make your own syringes or did you order them premade? its also possible there was some thermal damage to the syringes in transit and the spores contained, or a weak spore load in the solution. on your second set of cakes was your strain/supplier different?

I got both syringes from PF. I just asked for psylocybe cubensis spores, so I guess I got the basic ones. What are they? B+? I didn't even look at the tape on the syringe.
