My friend uses Malt Extract agar from fungi perfecti. It has a touch of antibiotic in it. He then supplements the malt extract agar with brewer's yeast, a touch of gypsum, spirulina and dog food. Lately he's also tried a little colloidal minerals that seems to spice it up nicely. He uses a small squirt from a syringe, and then rolls it around. Within 4-5 days @ 75F there's usually a dot started. He uses pre-sterilized petri dishes. He mixes the agar shit (it doesn't want to dissolve, but you can eventually wear down its resolve to not become part of the solution) with boiling water. Then boils it and stirs it to make sure it's dissolved. Then PC's it for 30 min. Then in front of a cheapo HEPA from W-Mart, fills the dishes. They're cool after about two hours (unfortunately he's still not cool). Then again in front of Mr. HEPA, with the petri dishes closest to the flow, he opens (tilts) the side of the dish closest to the HEPA and gives it a squirt. So far, the mycellium grow like crazy. I'm attaching a bit from Stamets w/o authorization.

I normally use the MPG [malt peptone grain] formula right out of TMC [see greymule's attachment] and it works like a charm. Only variation is to substitute rice flour for the rye if I'll be innoculating into rice based substrate.

If you start with spores you'll get fuzz 99% of the time. I almost always start with tissue from the base of the stem. Doing this at least 75% of the dishes are nice and rhizomorphic, rest are usually "intermediate", very few fuzz balls. Tissue from up in the cap seems to take off and grow faster, but fuzzes more.
