150L Autoclave - Hit and miss results….HELP!

I’ve recently switched from using stove top pressure cookers to this swanky machine however I am having trouble with contam. I’m going to give my process and if someone can see where I am going wrong that would be amazing.
Soak grain 20-24 hours Bring to boil Sit for 10 minutes Strain and let cool (approx 40-60 min) Put grain into bags Pack trays Pressure cook Settings are 122c 3 hours Maintains pressure until 90c then vents
I usually take out the next day and seal around 40c there a lot of condensation in the bags and grains look rather wet (look normal 2 days later).
Inject with LC up to a week later

They shouldn't be that wet after sterilizing and to dry up that much after 2 days is a little concerning.
What a fancy cooker, other than the grains being too wet I don't believe the issue is with sterilization.
Are you sure your LC is clean?
Maybe an obvious one but sealing the bag without letting in any air that could cause contamination?
I only say this because you can have great success with an insta pot for a PC if your LC is clean.
Do the bags contaminate without you inoculating them?

The bag on the left is straight out the pressure cooker still warm the bag on the right is after a couple of days at room temperature.
I had thought there was a problem with lc however the jars look fine and I have checked with agar no sign of contamination.
Some of the bags contam without being inoculated.
I’ve used one of these pressure cookers for years no problem.

The only thing that’s changed in my process is I have this big machines now and a bigger flow hood. I know the flow hood is fine so it must be the autoclave. Over the last 7 years anything I’ve needed has been asked on here and I’ve not needed to post. I’ve found maybe 3 posts about one of these on here and none of them are relevant.
Was hoping someone would have some experience with one and tell me if I’m doing something wrong.

you said your jars all come out fine. could the autoclave be getting significantly hotter than pressure cookers do, compromising the plastic of the bags? could 3 hours not be long enough to heat the center of the bags through?
