Unpredictable Behavior Under the Influence of "Magic Mushrooms": A Case Report and Review of the Literature

Abstract Fatalities implicating psychedelic mushrooms are not a common clinical situation in everyday forensic medicine. Despite classification as an illegal drug in many countries, psilocybin mushrooms have the reputation of being safe. We report the case of a young man who jumped from a second story balcony under the influence of psilocybin mushrooms. The psilocin assay was performed by gas chromatography coupled to an electron-impact ionization time-of-flight detector (GC-EI-TOF) after solid-phase extraction. Total psilocin was quantified in peripheral and cardiac blood as 60 and 67 ng/mL, respectively, and in urine (2230 ng/mL), bile (3102 ng/mL), and vitreous humor (57 ng/mL).
This case report and review of literature highlights the danger of psilocybin mushrooms. Isolated use of psilocybin mushrooms by a regular consumer without psychiatric history, even under "safe" circumstances, can lead to a fatal outcome

I think this more highlights the danger of jumping out of 2nd story balconies.
Suicide and drug use is a disturbingly common occurrence, particularly with alcohol.

Things like high places, busy streets and areas next to deep water are dangerous for people with altered perceptions.
I know of several deaths involving psychedelics where the subject drowned, fell to their death etc. LSD, DMT, Mushrooms, Mescaline etc, all of these entail that risk. Most users seem well informed about this risk, but not all.
I think that teachings like those from Carlos Castaneda, like his claim that a Shaman can jump from a cliff and not die because they turn into a crow and fly away, act as a type of suggestive poison that endangers vulnerable users of psychedelics.
High doses of psychedelics can lead to functional blindness, which is potentially dangerous. I've had to physically restrain people for their own safety. It's quite unpleasant and those of us who have seen this type of thing repeatedly have a strong reluctance to be around people taking high doses or who are tripping in irresponsible settings.
I still consider psychedelics safe for most people when approached properly.

Outside of falling off a balcony, which copius amount of alcohol could do to, the truth is some people are completely unprepared for the experience, they fear the toxicity and don't understand the LD-50 or they attribute their experience to deity like phenomena rather than the drug itself, with properly informed people, this doesn't happen.
With no further information on the case example, it's hard to say what influences or variables were truly at play.
