End of Life Trip Help

Hi All
A friend who is on hospice (any day now) for cancer, is on high dose of Dilaudid, a high dose Opioid for extreme pain.
He recently took 4g dried, but felt only minimal effects.
Does anyone have resources or links to the effects of opioids on a trip? Can a higher dose break through the pain meds? This is an immediate need question, would love any help.

Im on opioids and I need higher doses. But I would try 4g dry from another batch or type first. Everyone's different, but if he's on a high dose it could ruin it for him I'm afraid, I guess it's intended to help remove the fear?

The Johns Hopkins study was very successful apparently but I would assume that getting the dose and setting right is important along with also having medical training and knowing what you are doing (medical interactions, etc). I suppose they would also have had trained psychologists/psychiatrists on standby as well along with trained facilitators/guides in case people got into trouble (as surely some must have)?
A very laudable effort on your part but you might be best tracking down someone who was actually involved with the study and learn a bit more about it. If you are not doing it with the hospitals knowledge, and shrooms are illegal where you are, you could even get into serious trouble.

I believe that is the point that many people miss when looking at the Johns Hopkins Existential Anxiety study. The participants where monitored by two professionals during their experiences. I do not think that a normal healthy individual can have more than their imagination run wild, which can be terrifying. For a person that is possibly on multiply medications and in a weakened physical state there could be some danger.
I do not know the law where this person is but administering or supplying a substance that causes a lethal reaction is something to consider.

i don't think salvia is the right thing to do before you go 
yea opioids suppress psychedelics for sure.. take more mushooms then. or lsd might work and also feel better

do you, and i mean your friend too, realize that if he gets a potent trip, his body might seize the opportunity to die during that experience?
if you say "any day now", a strenuous trip might literally be the ticket to the other side.
If it were my death bed i'd want it to be Ketamine.
K would also strengthen the painkilling effect and lower dilaudid tolerance.
Divide your energies well, it will be hard on you,
Dissoiatives like K would dissociate him away from both his pain and his death anxiety. This is why its given on the roadside of accidents by paramedics (and battlefield medics since vietnam) he should asl his doctor, low dose death bed ketamine makes a lot of sense medically.

I also second Ketamine, but add LSD in. Have him take the lowest dose of opiates that day he's going to trip to still be comfortable, but hopefully less than normal. Dose him with 150ug of LSD. Around three hours or fours in, give him an IM injection of Ketamine of like 50-60mg.
He'll have a breakthrough on that, and it won't be a dangerous combination with the opiates. But like Asante said, he may very well choose to die during the psychedelic experience. It wouldn't kill him per say, but he would choose to leave and not come back.
High doses of opiates will absolutely dull psychedelics.

If I was on end of life hospice, I would go out on a combo of lsd, ket, shrooms, and mescaline. It would be a wild ride out, but I would manage. Probably take a huge hit of dmt on one of my final breaths

Which isn't necessarily a bad thing, but should be something discussed at length.
Ultimately the decision is up to him, but if it were me, I would want at least lsd and ket. Like 800ug and 800mg. Full dissociation and mostly closed eye visuals with kaleidoscope fractal effects
