It looks to be rotting from the roots up. It should not be lying down like that, have you been overwatering? Looks like the roots have not established themselves and are soggy.

cacti will lay down sometimes when stretching twards the light, and/or sometimes when under-watered.. i think i see a window.. maybe it was stretching twards the window? and you turned it for the picture? i don't see any rot, it looks like it's doing good, just needs more even lighting, or a daily turning.
i'd let the tip grow for a few more inches, and then make 3 new cuttings...
it looks like bridgesii.

Maybe I'm not seeing it correctly but cactus will grow toward the light, not fall over like that.

I would repot it and give it more sunlight.

I was thinking maybe peruvian torch?
Needs repotted with high draining soil getchu some high quality cactus soil on ebay or etsy
And more light might help etiolation or growing sideways

good point, it looks more planted sideways, rather than growing sideways twards the light.
