I just had the same problem. I did not make a even layer of w/ my cakes just the top was even. Make sure the mycel is evenly placed and covered evenly. I am going to try cutting them into slices next time and have it all the same depth instead of chunk here and there.

Deep scratching helps... but also using a good initiation strategy. As the casing surface is colonizing, in the dark, dont fan it as much. Allow more C02 build up then usual.
After about 7-10 days, and the casing surface is looking good, start the 12/12 light cycle and start giving it lots of fresh O2. This will help a lot with a good pin set and even flushes.

Listen to what Ryche said, but if you want to deep scratch, be absolutely sure there are no contaminants on your casing layer. How do you know this if they aren't visible? If you're not using some good method of air exchange, I'm willing to bet your casing layer SHOULD NOT be scratched.

Deep scratching and scratching are two different things. Deep scratching goes all the way down to the substrate layer, and if you do this, your casing wont pin for another week or so following the deep scratching "procedure". As I've once heard it spoken somewhere "Treat scratching like a brand new casing layer."

Even casing layer, even substrate layer and what Ryche said--those are your most likely avenues to good pin-set.
