Growing But Failing to Complete

This is my third set. Within my first two sets I've been able to get 6-10 successful flushes.
However with this set, B+, I've only reached 2 successful flushes. The mycelium seems to still be active, but the mushroom doesn't complete its growth before losing steam and going bad.
Any thoughts on why this could happen? The only thing I can think of is colder temperatures in the house due to winter. When using heat my home is 68-70, whereas during warmer months I have the A/C kick on at 74-76.

surface conditions dont look so hot. Or better, it looks very dry
at this point I'm not sure its gonna bounce back. Do you have pics of the spawn? What substrate did you utilize?
temps sound fine imo, I keep my grow room around 70
I think I see matting? Did you notice it was dry and soak it down at some point?

Thanks for the responses. Just doesn't make sense because my first and second batches hit 5-10 flushes. These only lasted 2.
The substrate is still moist.
I used an all in one bag, similar to first two attempts.
This could technically be my 4th flush of the batch. 1-2 produced and 3rd stalled like this. I removed aborts and dunked. And it continues to pin and start going. Thats why it doesn't seem like its out of energy to me. But then again I'm very new to this.
Whats matting? Sounds like your assumption is that I let it dry out (formed a layer) and then tried to revive it by rehydrating it. I'm not saying that may be accurate, but what the visual indicator that makes you believe that? Trying to learn.
Yes, the substrate may not be in ideal conditions if I've left some tiny aborts remain and then dunked.
