LF sport print trading liabliity form/microscopy use only form

does anyone have a link or a scanned image of a microscopy use liablity form that i could use to send with prints i mail off to fellow mush friends on here?...ive seen other ppl mail em to me with whatever print they sent and i had my own made up but it got lost to the sands of time..can anyone help me out or point me to the right place

I've never heard of a form like this, and I doubt it would do anything other than make the prints look more suspicious.

i recall a shit ton of ppl years ago sending for microscopy use only forms with every print they would mail out

They still do. But it's just a piece of paper that makes them feel safe. It has no real legal value or meaning.

Microscopy use only.
Or
Not intended for illegal purposes.
That kind of shit. Nothing special.
