Requesting Help with Inoculation Problems

Hi! Lately, I've had to toss a lot of rye wheat berry inoculation bags and any insight into what I'm doing wrong would be very helpful.

Problem Agar dropped into an inoculation bag either doesn't take or takes, but then fails to spread after a shake-up.
Tek Rye wheat berries soaked in water for 21-24 hours. Brought to a boil, laid to dry. 5lb in a plasic bag with 1" hole covered with breathing tape. 90m in PC at 11 PSI (best I can do) Let cool overnight.
Drop agar slice into bag in SAB. Seal with tape and set aside.
Outcome 1: Agar dries out and nothing takes. Outcome 2: Myc forms. 10 days in, I shake and let sit. Check back in a few days and looks diseased.
I think the problem is proper hydration of the wheat berries and/or bursting. When exposed to air after the shake up, the burst berries mold and such.
In the first two bag pics, you see the myc actually backing AWAY from where it had originated with a yellowish ring around it.

in the first bag pic, the left section is myc the right section is mold.
90 min @ 11psi probably wasn't enough... I do 2 hours at 16-17psi. Some people do 90min @ 16psi for smaller grains but oats and such can be pretty grody so I always do 2 hours. Also when using bags instead of jars that's a lot more volume to sterilize so the heat/steam might not have penetrated to the center of the bag as well as it would have jars.
I don't use spawn bags but you might want to look into buying premade ones or a better tek for making them yourself, I don't think micropore tape is a good filter for PC'ing grains, it works for no-pour agar plates because of the low PC time but I don't see it working well for anything else.

In my first grow with this method, I made 10 bags and had to toss 2. In this grow, I am following the same tek and made 6 bags and through away all 6.
Your comments make sense as my smaller mason jars generally don't have this problem (until the myc reaches burst grains at the bottom). I'll ditch the bags for inoculation and switch to jars and use smaller fruiting containers.
