Grains Exploded in the PC

I’ve got some weird stuff going on with this batch of grains I cooked up yesterday.
There’s two things going on that I’ve never seen before, and I’ve cooked up several batches of grain recently. I’ve been following Bod’s oat prep tek and I’ve had good results. Haven’t lost any jars recently and have several monotubs going.
I cooked them up yesterday and they sat in the PC for about 24hrs, I let them sit in there for that long almost every time I do grains. I have a 23qt presto that hold 10 jars, 7 upright and 3 on their sides up top. As I removed the top 3 I noticed there was some strange discoloration on the tinfoil of one of the upright jars

I continued to remove the jars and shake them to break up the grains one by one. They all seamed normal. I went to grab the second to last jar and was surprised to see the grains seam to have exploded/melted in there!

There was a few things that were unusual about this cook session. One thing was that I didn’t have any gypsum in this batch of grains. I usually add a heaping tablespoonful every batch but I just ran out and haven’t got any more. The grains seamed just slightly more sticky in there but they still broke up as usual. Not a big difference.
Also I’ve finally started using added weight to quite the jiggle weight on my PCs. I use 2 quarters secured with micropore tape. According to Bod’s tek he says cook at “15-18psi”. There was one time with this batch were the pressure gauge on one of the PCs got really close to 18 and was up there for a few minutes but never went over 18psi. I usually manage keep it 15-17psi the whole time so that was unusual. Overall they cooked just a little bit hotter than normal but still well within the 15-18psi range.
Other than that everything was normal and the other jars seam okay... also not sure what caused the stuff on the tinfoil though, are they related?! The jar with the melted grains was NOT the one with stuff on the lid either...
Jar failure? Lid? Tinfoil? I should probably toss these grain in the compost pile rather than inoculate...
Any input would be greatly appreciated. Anybody seen this before?! Also any advice on the modified jiggle weight? What do you use for added weight and how much?
Edit: I just shook the weird jar and it’s like the whole bottom 1.5” was fused together. After shaking it left a film on the inside of the jar

Well this is an odd one! 
I believe the discoloration on the tin foil might be caused by rust/algae on the lid of your PC that dripped down onto the jars.
As for the burst grains, its probably from the PC getting to 18PSI! I would take those quarter weights off.

Just from a purely "self preservation" point I'm seconding the removal of quarters. PCs are no joke, modifying them in any way is no bueno imo and just asking for bad juju.

This is what my PC lid looks like. I always add a tbsp of white distilled vinegar. It’s fairly new with only a few dozen cycles on it at most.

Yeah the jiggle weights are a new thing for me. I used to have a PC with no pressure gauge, just the jiggle weight. It was always very simple to just reduce the heat and let it cook for 2 hours at 15psi. I recently upgraded and now have 2 PCs with the gauges. That’s when I started adding weight as some people recommended to stop the jiggling and reduce the noise. Now I find myself constantly tweaking both burners as they slowly drift up or down within the acceptable range it is more quite though

You’re probably right about that, but I always thought that’s what the black safety plug was for! I just thought it would pop off like a cork on a champagne bottle if the thing ever hit 20+ psi

In theory, yet if you are investigating the problem when it blows then you're looking at a trip to er and at least a few skin grafts

I never considered the risk before but yeah, definitely wouldn’t want to find out
edit: I just revisited Bod’s “Pressure Cooker TEK” and he specifies that he uses 3 quarters as added weight.
