What's going on? Debug my grow (novice)

I dunno what's going on here. Obviously having been a member here so long I should know a thing a two, but I don't mind admitting when it comes to this side of the hobby I'm a total novice (I'm pretty good hunting edibles though). 
A picture says a thousand words, so I've included a few. Here is the story so far:
That "cake" that the mushrooms are growing from was basically "the rubbish pile". I made a really nice, proper layered container really carefully with everything super sterile and lots of crushed up grain media and that is my "main grow" (the one that's not growing a single thing). The left overs I just threw on one side with the un-sterile scraps of grain media and substrate left-overs and chucked it in a gardening pot and mixed it up with my bare hands. But it's the "left overs" that are actually growing mushrooms! The "main grow" you can see ... it's full of snow-white awesome looking mycelium but it just refuses to pin. I've cold-shocked it twice, changed the fruting chamber, changed the heat, changed the light. Nadda. But the left-over "refuse pile" ... that decides to grows mushrooms (albeit only 3).
So a few questions are in order:
1. What do I have to do to get the "main grow" to pin?
2. Why on earth are they growing sideways?
3. There was lots of "aborts" happening on the "left-overs cake" ... why is that? How do I stop them aborting?
4. Why are the stems so long and skinny? The look weird compared to the ones I've found in the wild.
FYI I used the "micro-pore tek" in the shroomery cultivation guide to make the fruiting chamber. They are exposed to light ... not on a schedule or anything just when I wake up and go to bed I turn a small desk-lamp on them. Maybe that's not enough light?

So if i'm getting this right your main grow is the larger pyrex dish? Seems like you need to have better FAE, evaporation triggers fruiting. For you scrape pile however you've experienced the tenacity of fungus, Take spore prints for the next generation.

Yes! The "main grow" is the last picture of just a tub full of snow white mycelium and no pins. Can it be too wet to pin?
Yeah I actually considered that, there's not heaps of fresh air getting there, I've been fanning them a couple of times a day and that's it.
And they were actually in just normal sealed tupperware before, so they were only getting air when I fanned them.
Like I said, I thought it might be FAE so I made the "micropore tek" tubs today and moved them into that today. We'll see if that improves things...

  Here's one of my tubs. Pics of yours? Its important to allow CO2 to escape, its heavier than air so you need holes lower on the container. You also need to allow fresh O2 into the tub as well.

Nice one! Do you have nothing at all covering the holes?
So this is the one I made today ... it's actually bigger than it looks (weird perspective) but it's still not big. Small-medium sized.
The holes in it are 1.5 inches. The top holes have one layer of micropore tape and the bottom holes have two layers (one inside and one outside).
I also made some more substrate (I'm leaving it too cool overnight). In the morning I will crush up the "good grow" and mix it with the new substrate and give it a new home in this tub.

I have all the holes covered with Micropore tape. I am unsure of the kind of tape you're using, usually we use the medical 3M micropore tape. The tape you're using there I think could be an issue. I also suggest polyfil, its a bit easier to dial in for a tub that tiny. Also make sure your myc will actually fit this container, you may need to use a liner for side pins. I also suggest that next time go for just a regular shoebox.

The tub is a bit more than 1 foot across. It's not as small as it looks in the picture.
The mircopore tape I have used is 3M tape.
I don't understand what you mean by the last two sentances. I don't know what "myc" is or "a liner for side pins" or a "regular shoebox".
You see how much I know about this.

    Here's a regular shoebox, no holes, no planning, just toss in spawn/sub and presto.
myc: Mycelium/mycelia - the vegetative part of a fungus, consisting of a network of fine white filaments (hyphae). Side pins are fruits that pin on the side or under the substrate and are a pain in the ass to harvest and remove without damaging your grow. You can use a black bag as a liner to prevent these if your substrate is really deep.

Looks good. Must just be a lack of fresh air in mine I think.
BTW, I have painted the bottom of the box black r.e. side pins.

Whatever works for you m8. The black painted bottoms never actually worked for me no matter how I did my substrate. You'll come to understand that its how you pack your tub, pack it right you don't need a liner or the blackout paint job. Tamp down your substrate and use at least a pseudo casing layer of Coir and verm.
