Reason for mutation? (2nd cap growing directly on top of 1st cap)

So first of all an acquaintance of mine thought these may have been some early birds since it looks like their sub could definitely be further colonized,

however they still see more pins forming/hyphal knots all over.. Plus they already harvested a mature fruit from this tub that was basically just like any other fruit they would harvest from any of their other tubs but then it became glaringly obvious mutants were taking over



So my acquaintance would appreciate any answers to the following.....

1))  Are these early birds ?and if so should my acquaintance return the tub back to colonizing conditions or not ? (after seeing these and the first healthier looking mushy they moved the tub to fruiting conditions)

2) What would likely causes be for the emergence of ‘early birds’ ?

3) what causes fruits to mutate in this fashion (if not because they are early birds then I’m definitely curious why this happened... but even if premature fruiting is the case I would like to know what about early bird conditions would result in such a dramatic change)

I know I didn’t need a new post for questions 1&2 but as for #3 I havent found anything on this site with regard specifically as to why one mushroom’s cap would start having another cap grow out of it.
